Se presenta el caso de una paciente de 28 años con diagnóstico de d-transposición de las grandes arterias corregida con técnica de Senning, quien fue remitida para la ablación de un flutter. Se realizó reconstrucción anatómica 3D mediante el sistema de EnSite NavX®, y se confirmó la participación del segmento posterior del istmo cavotricuspídeo, correspondiente al canal arterial sistémico, en el circuito del flutter. Se realiza ablación y se objetiva la terminación del mismo con una lesión focal.

A 28-year-old female with a diagnosis of d-transposition of the great arteries (d-TGA) and a physiological correction with Senning’s technique, was referred for a radiofrequency ablation of an atrial flutter. A 3D anatomic reconstruction using the EnSite NavX® system was performed. Mapping of the flutter’s circuit proved the participation of the posterior segment of the cava-tricuspid isthmus (systemic atrial channel) in the mechanism of the tachycardia and a termination of the flutter was observed with a focal lesion.
